my rear disc brake is a little loose.. the part that acctually grips to the disc.. not the disc itself. Is there anyway of tightening this? does it happen often?

hey mine is loose too....so is the locknut...its all the way tight. also, how do i go about changin my bearings? i have some new bearings on hand & some dust seals...is it that hard to do?
