chain keeps getting loose

on my 250x my chain gets loose really quick (10 min ride) i tighten it and it just gets loose again it has new rear carrier bearings. what could be the problem? o and the chain comes off too.

either chain is old and worn out, swinger bearings/bolt is junk, or both

Try a new set of sprockets and a new chain.

Make sure your pinch bolts are tight.
