What kind of carrier is this?

I bought this on eBay a while back, the guy selling it didn't know the brand either. I never used it, I sold the bike I was planning on using it in. Bearings are smooth, and its in great condition, just don't know the brand.

Model number "H-10" is the same, yours may be an older design?

I've seen others on ebay that have the "H-10" engraving, and they are not RAD. I think there might have been a guy called ajc racing on ebay for a while. RAD carriers have a hex center section, but they are about the same other than that.
