Do the crank bearings go out because of wear or from poor mantiance

Yes is correct. They can go out for no reason but keep up on oil changes and you'll be better off. The big thing is by changing the oil often, you will see when the bearings are about to go and you can swap them out instead of waiting for them to go and tearing **** up.
