If you are going to an AMA event, most of the time you can signup for it there. I know for sure, you can do this at a GNCC and a couple smaller series. I'm guessing you can also do it at an ATVA MX event.

you can sign up for AMA at any District 13 or 29 Race in VA and NC. I'd imagine that holds true for any mx race.
