Sounds like he has some broken teeth on the gear or the shift fork might be a little bent causing it to not fully engage into 3rd gear. I have a full transmission for a 250X if you need some parts

i have seen end play of the shift drum cause this. i'd also have a look at the ears on the dog gears.

The '87 250X's had a 3rd gear problem. I had the same problem. You will have to tear it down and replace 3rd gear, the gears on both sides (2nd & 4th?), the shift fork, and I think I replaced the shift drum too. All the parts you get for it today, if you order new, will be updated. I didn't have any problems after I replaced them. It's a big job. If your mechanically inclined, you can do it yourself. I recommend buying a factory Honda service manual. Take your time and take lots of notes on how you take it apart. This will help when re-assembling. Good luck.

had a 250x years ago and also had to change the gear and shift for if i remember correctly. fixed all the problems and sold later to my brother in law who still has it and runs and shifts perfect. but splitting the cases is not the funnest thing to do.
