Are the 250r/400ex rear rotors the same bolt pattern?

had to do it..... but as to your ?, I do think they are interchangable. But that is just a gut feeling I have. IF they are, I think it is just the 87-89 250r (or was it 85-87?). You may want to check a bit further.....though my hunches are pretty good.

250r rear brakes will fit on a 400ex. You have to do a complete swap (rotor, caliper, brake stay). I've also seen people use 450 rear brakes on 400ex's. Main advantages are dual piston caliper=greater stopping power and smaller rotors= more ground clearence.

the rotors are not interchangable. 85-89 250r's use the same brake parts except for the one year only 85 caliper mounting bracket and brake stay. Yes, for 250r, 400ex or 450r you have to swap the complete parts, none will interchange.
