Chain slack

I need some help we got a polaris 90 and were having trouble with the chain coming off. We have been using a roller chain tensioner and it still comes off sometimes. I took off the roller and I noticed the chain goes from tight to loose and back to tight within one rotation of the sprocket. Frame is not broke , swing arm dont look bent and both sprockets seem to be lining up. Any help or pointers would be great. Thanks a lot Joe

Check for slop in axle bearings or worn out carrier. Also teeth on sprocket. make sure all ok first. Thencheck into ZTR chain tensioner.
