loose axle

my buddy z has loose axles, does it need new bearings? if so is it very hard to replace?

make sure the axle nut is not loose, if they need to be changed it is straight forward wrenching no tricks or specail tools but i can't remeber if the axle nut are oppisite thread or left handed anybody?
