426 timing chain tensioner keeps breaking

i have a 2002 400ex bored to a 426 and my timing chain tensioner has broke twice. the first time i didnt know what the dam thing broke i didnt know it and the chain broke too. WHY IS THE TENSIONER KEEP ON BREAKING???????

Are you retracting the tensioner before you install it? Or are you just forcing it in with the bolts?
